The medical practice of Dr. John R. Shrum, M.D. has arranged to transfer his patients’ medical records to Charlottesville Dermatology, 600 Peter Jefferson Parkway, Suite 230, Charlottesville, VA 22911 (434-984-2400) following his untimely recent passing. Pursuant to Virginia Code § 54.1-2405, at the written request of a patient or an authorized representative of the patient, copies of such records will be sent, within a reasonable time, to a physician of the patient’s choice or to the patient. A fee that shall not exceed the actual costs of copying and mailing or delivering such records may be charged.
