July 15, 2011 — Notice is hereby given that University of Virginia Medical Center has acquired Hematology Oncology Patient Enterprises, P.C. (“HOPE”). As a result of the acquisition, HOPE patient medical records will technically be transferred to UVA Medical Center but will remain physically located where they are now with the same physicians that previously provided and will continue to provide care.

In accordance with Virginia law, we are providing notice stating that, at a patient or an authorized representative's written request, records or copies will be sent within a reasonable time to the patient, any other physician or medical practice in accordance with Virginia law. Charges, not to exceed the actual costs of copying and mailing or delivering records, would be made for supplying the patient or provider with the originals or copies of medical records.
